EFNEP collects demographic data on all youth groups who participate in the program. In addition, questionnaires are administered at entry and at exit to measure improvement in knowledge and skill development for children and to measure improvement in behavior for youth.  A Youth Evaluation Tool Committee has designed, tested and implemented youth evaluation tools that assess outcomes experienced by children who participate in the Youth Expanded Food and Nutrition Education Program (EFNEP). The evaluation tools are for four different age groups:

  • Grades K-2
  • Grades 3-5
  • Grades 6-8
  • Grades 9-12

Links to evaluation surveys, and frequently asked questions about the tools and the sample forms are below.
